“Up to this point, all therapy for lupus has been a blunt instrument. It’s widespread immunosuppression,” Jaehyuk Choi, a medical doctor and Northwestern Medicine dermatologist, said. “By identifying the cause of this disease, we have found a potential cure that will not have the side effects of current treatments.”
The study explains how lupus develops, pointing to abnormalities in the immune systems of people with the disease.
“We have identified a fundamental imbalance in the immune responses exerted by lupus patients, and we have defined specific mediators that can correct this imbalance to reduce the pathological autoimmune response,” said Deepak Rao, a said the medical doctor and assistant professor of medicine at Harvard Medical School.

“We found that there is this fundamental imbalance in the types of T cells that are made in lupus patients,” said Dr. Deepak Rao, one of the study’s authors and a rheumatologist at Brigham and Women’s Hospital in Massachusetts. T-cells are white blood cells that play an important role in the body’s immune response.
The study reached its conclusions by comparing blood samples from 19 people with lupus with blood samples from healthy individuals. The comparison showed that healthy cells in people with lupus had too many of a particular T cell associated with damage and too few of another T cell associated with repair.
At the heart of this imbalance lies a protein called interferon, which helps protect the body from pathogens. Scientists have known for years that people with lupus have higher amounts of type I interferon — but the new study links the issue to a number of negative effects.

Blocking this receptor hinders the production of T cells that can help heal wounds on the skin, lungs, and intestinal barrier. It also stimulates the production of T cells involved in making autoantibodies, which attack healthy cells and are the hallmark of lupus.
Rao said the theory could explain most cases of lupus.
“I think this will apply to essentially all patients with lupus,” he said.
“This is very exciting research and very promising, but I think it’s too early to say that this is the root cause of the disease,” said Mara Lennard Richard, scientific program officer at the Lupus Research Alliance. The alliance is a private funder of lupus research and has contributed grant funding to Rao’s study.
Because the symptoms of lupus are so diverse and the contributing factors are manifold, “it is very difficult to find a root cause for the disease,” said Lennard Richard. “Obviously, if it causes lupus, that would be amazing and really fantastic for people with lupus.”
Dr. Jill Byne, director of the rheumatology department and Lupus Center at NYU Langone Health, said the theory will need to be tested in a larger sample of people.
